Physical Therapy Initial Evaluation

This is a comprehensive physical therapy evaluation and assessment. We will begin to determine the root cause of the dysfunction. There will be special consideration for any clues the body is leaving in regards to systemic inflammation. You will be provided education and instruction on an appropriate home program. You will also be educated on any integrative approaches that may aid your healing (nutrition, nervous system, circadian rhythm, etc…)

Physical Therapy Treatment

55 minutes, 1:1 sessions

$165

We will continue to progress and address: dysfunctional movement patterns, joint and tissue mobility, strength and nervous system feedback. You will be educated each step of the way so that you understand the plan of care. You will also be provided updated instructions and home exercise program after each visit.

Feet and Shoe Orthotics

15-30 minute add on service if needed

$85 (cost of insoles) + time

Jen has a special niche with feet and toes since they are the foundation for all movement and propulsion.

This service includes an evaluation of your feet and recommendation for semi-custom foot orthoses (shoe inserts) if they would benefit. You can typically leave with insoles the same day. The insoles will be custom molded to your foot. She will make recommendations for shoe wear or any additional tools as needed.

  • Jen is a concierge, out-of-network, provider meaning that she does not directly bill insurance. The benefit of not billing insurance directly is that 100% of your visit is devoted to your individualized needs and not dictated by insurance parameters, authorization forms or what codes/ services they will cover. She can work to correct the full body (and get to the root of the dysfunction) without being isolated to one body part. Since your session is 1:1 and devoted specifically to you, she rarely sees patients for more than 6 visits.

    Jen will gladly send you super bills after your physical therapy treatment for you to submit to your insurance company. You will want to call the number on the back of your insurance card to ask what your out-of-network coverage is for physical therapy and how you submit for reimbursement. Some insurances are simple and have an online process or fax, while others have forms. There are no hidden costs and payment is due at time of service. FSA/HSA cards are also accepted.

  • In the state of Missouri, we do not need a physician referral for the initial physical therapy visit. If subsequent physical therapy visits are required, we will fax a prescription with a detailed evaluation to a provider of your choice.

    If we are seeing you for wellness/ injury prevention, performance or environmental coaching- no physician referral is required for coaching services.

  • We can ask your provider to order specific labs or I can assist and order specific tests and test kits through Great Plains Laboratory, Vibrant America/ Vibrant Wellness, Microbiology Dx or Envirobiomics. If you require imaging, we will contact your care team.
